Serum CTX Test for Bone Loss: What It Measures and Why It Matters

What Is the Serum CTX Test?

The Serum C-telopeptide (CTX) test is a blood test used to measure bone resorption, or how quickly your body is breaking down bone tissue. CTX is a marker of collagen degradation, specifically from type I collagen, which makes up the majority of bone.

This test is especially valuable because it gives us a real-time snapshot of bone turnover, often before changes appear on a bone density scan (DEXA). It is offered through LifeLabs.

How the CTX Test Works

CTX is a biochemical marker of bone resorption, it measures the fragments (called C-terminal telopeptides) released when old bone is broken down. These fragments circulate in the blood and can be measured to assess the rate of bone loss.

CTX levels are typically highest in the early morning, so we recommend testing after an overnight fast, ideally between 7–10 AM for consistency and accuracy.

The CTX test helps us:

  • Detect early bone loss, even before it shows up on imaging
  • Assess risk for osteoporosis or osteopenia progression
  • Monitor treatment effectiveness – whether you’re using medications, supplements, or lifestyle approaches
  • Track response to interventions like hormone therapy, resistance training, or nutrient protocols
  • Guide decisions on calcium, vitamin D, magnesium, vitamin K2, and other bone-related therapies

Who Should Consider a CTX Test?

We commonly recommend CTX testing for:

  • Postmenopausal women at risk of osteoporosis
  • Individuals with osteopenia or osteoporosis looking to monitor progression
  • Those on or considering bisphosphonates or other bone-active medications
  • Patients with chronic inflammation or autoimmune conditions affecting bone health
  • Athletes or over-exercisers with hormonal imbalances
  • Individuals on long-term corticosteroids, proton-pump inhibitors, or thyroid medications
  • Anyone with fractures at low impact, suggesting reduced bone strength

Interpreting the Results: What We Learn

CTX values vary by age, sex, and menopausal status. We compare your results against standardized reference ranges to assess bone turnover rate.

Elevated CTX may indicate:

  • Active bone loss or resorption
  • Inadequate bone-building activity
  • Hormonal deficiencies (e.g., low estrogen or testosterone)
  • Nutrient deficiencies
  • Chronic inflammation or oxidative stress

Lower CTX levels may indicate:

  • Stable bone remodeling
  • Positive response to therapy
  • Reduced bone resorption due to medication or lifestyle changes

It’s important to interpret these values in context, we never look at CTX in isolation. Your DEXA scan, vitamin D status, hormone levels, inflammatory markers, and lifestyle all play a role in the full picture.
